Beneficiary Designation

A beneficiary designation for Equity Foundation is a simple and meaningful way to express your values and create positive change for future generations of Oregonians. Designating Equity Foundation as a beneficiary is easy and does not require an attorney. All you need to do is contact your insurance company, employer, or the company that manages your retirement plan, request a beneficiary designation form, complete it, and return it.
